Referring Patients

We would like making the referral process as easy as possible, and we appreciate your support of our new practice.

We have completed the re-credentialing process for most insurance plans. Caresource is not accepting new doctors because their panel is closed.

When you call, we will ask for the patient’s demographic information including insurance and a brief summary of the problem (a copy of your exam and/or this form). Once we receive this and confirm the patient’s insurance eligibility, we will call the patient to schedule the appointment.
